Gonadorelin
Gonadorelin Acetate (Synthetic GnRH / LHRH)
Synthetic decapeptide identical to endogenous gonadotropin-releasing hormone (GnRH/LHRH). Historically FDA-approved as Factrel (NDA 018123, diagnostic test for hypothalamic-pituitary function) and Lutrepulse (Wyeth pulsatile pump for primary amenorrhea; both discontinued in the US market). Currently NOT available as an FDA-approved finished product in the US: dispensed exclusively through 503A compounding pharmacies. The 2023-2026 trend is sharp: TRT clinics (Defy Medical, Marek Health, Alpha MD, IAM Clinic, Full Potential HRT) are increasingly switching from HCG to gonadorelin because (1) HCG supply has been unstable since the 2022 FDA compounding clarification (HCG removed from the 503A bulk substances list pending FDA review), (2) gonadorelin acts upstream at the pituitary preserving more of the natural HPG axis, and (3) cost is meaningfully lower ($30-50/mo vs $70-100/mo for HCG). Tradeoff: gonadorelin's ultra-short half-life (~10-40 min terminal) makes it less effective at maintaining intratesticular T and spermatogenesis in deeply suppressed men. HCG is still preferred for fertility-priority patients.

Mechanism
GnRH receptor (GnRHR) agonist on anterior pituitary gonadotrophs. PULSATILE, low-frequency, brief exposure → physiologic LH and FSH release → testicular T and spermatogenesis preservation. Unlike triptorelin/leuprolide (long-acting GnRH agonists that cause downregulation), gonadorelin's ultra-short half-life (distribution t½ 2-10 min; terminal t½ 10-40 min) mimics natural pulsatility and avoids GnRHR desensitization when dosed correctly: the pulsatile-vs-continuous downregulation distinction is established pharmacology. Current 503A compounding status and dosing patterns documented. TRT clinic industry-wide shift 2023-2026 from HCG to gonadorelin driven by HCG supply instability, lower cost, and upstream HPG axis preservation. Continuous infusion would cause the same downregulation as the long-acting agonists: bolus dosing is mechanistically essential.
Dosing
No standardized human dosing has been established. Every catalogued study for Gonadorelin is preclinical or a literature review. Amounts used in animal models do not translate directly to humans.

- Injection site reaction
- Common Mild · Mild local erythema/wheal: peptide solution. Resolves within hours.
- Headache
- Occasional Mild · Reported infrequently; usually transient.
- Flushing (high IV doses only)
- Rare Mild · Diagnostic IV bolus 100mcg can produce transient flush; not seen at typical SubQ TRT doses.
- Nausea

- Better tolerability vs HCG
- Common Mild · Notably ABSENT vs HCG: no post-injection nipple sensitivity, no direct E2 surge from testicular aromatase. Gonadorelin produces physiologic-pulse LH rather than HCG's sustained LH-mimicry: flatter E2 profile.
Interactions

Test Cypionate Synergy
Co-prescribed with TRT: gonadorelin preserves testicular function (volume, intratesticular T, partial spermatogenesis) during exogenous T suppression of HPG axis. The standard modern TRT add-on.

HCG Redundant
HCG mimics LH at the testicular level; gonadorelin causes endogenous LH release. Same downstream effect via different upstream mechanism: combining provides no additional benefit and complicates lab interpretation.

Cetrorelix Contraindicated
GnRH antagonists block GnRHR at the pituitary: completely abolishes gonadorelin's effect.

Triptorelin Contraindicated
Long-acting GnRH agonists cause GnRHR downregulation and chemical castration: opposite intent of gonadorelin's physiologic-pulse goal.

Opioids Caution
Chronic opioids suppress endogenous GnRH (opioid-induced hypogonadism). Gonadorelin can rescue LH/T in this context: emerging clinical interest.
Reconstitution and storage
Compounded as lyophilised acetate (2mg or 10mg vials). Reconstitute with bacteriostatic water. Typical: 2mL BAC water per 2mg vial → 1mg/mL stock, OR 10mL per 10mg vial → 1mg/mL. 100mcg dose = 0.1mL of 1mg/mL stock. Some compounders provide pre-mixed 2mg/mL solutions.
Lyophilised: refrigerate 2-8°C. Reconstituted: refrigerate 2-8°C, use within 28-30 days; protect from freeze-thaw and light.
